A DOCTOR'S REQUEST IS REQUIRED TO AVAIL THE VACCINE ADMINISTRATION
SERVICE.

-Please ensure you consult with a doctor before availing the vaccine administration service. Your doctor will assess whether you have any conditions or risk factors that may impact your suitability for vaccination, as certain medical conditions or medications may require special considerations or precautions before receiving a vaccine. If you do not have a personal doctor, HPD branches have doctors
available for consultation. Please call your chosen branch to schedule an appointment.

-Vaccines are available on an order basis through our third-party provider, MACROHEALTH INC., with a tentative delivery time of 5 to 10 working days.

-Availing of the vaccine administration service is deemed final and is not eligible for refund or exchange unless the vaccine is defective or in poor condition.

-Vaccination fee includes both the vaccine and the administration by one of Hi-Precision Diagnostics’ Primary Care Physician, provided that the patient presents a doctor's prescription specifying the vaccine type, dose, route, and timing required.

-SENIOR/PWD DISCOUNT IS ONLY APPLICABLE FOR WALK-IN TRANSACTIONS. PLEASE BRING YOUR ID FOR VERIFICATION.

-Not valid in conjunction with other promos, discounts and packages. In the purchase of goods and services which are
on promotional discount, in compliance to R.A. 9994 (Expanded Senior Citizen Act of 2010) and R.A. 10754 (Act
Expanding Benefits and Privileges of Persons with Disability), the senior citizen or PWD can avail of the promotional
discount or the discount provided under the said laws, whichever is higher and more favorable.
